In response to the local and national precautions being taken regarding COVID-19, CCE Franklin is suspending public program outreach that is considered non-essential until further notice.

Medical Home is individualized nutrition / physical education consultations through Alice Hyde Medical Center.

• Referrals are sent through a Primary Care Providers at local Alice Hyde Health Centers
• Once referred, Family Health & Well-being Educator, Laura Weber reaches out to establish contact for consultation
• Flexibility of appointment location
• Consultations cover goal setting, healthier eating habits and methods to increase physical activity
